Obtaining Your Records


UMC offers a dedicated Health Information Management team to provide you with convenient access to your medical records.


Please follow the steps outlined below to request copies of your medical records. 


BIRTH CERTIFICATE PROCESS


After your baby is born, UMC’s team members will provide you with a birth certificate worksheet to complete. This worksheet is required to receive your baby’s birth certificate from the State of Nevada.

You will be asked to validate the information on the worksheet before the request for the birth certificate is filed. Once the request has been filed, you will need to contact the State of Nevada for the birth certificate and to make any changes. You can reach the State of Nevada at 775-684-4177.

Obtaining Your Medical Records

Many physicians in our community have electronic access to UMC medical records. This may help eliminate the need for patients to obtain records themselves. In addition, existing patients have convenient access to information through UMC MyChart to obtain lab results and schedule appointments. To sign up for UMC MyChart,  please visit https://umconnect.umcsn.com/.

If you would like a copy of your medical records, please complete the Authorization for Release of Medical Records (click this link to download) (click here for the Spanish version) and forward the completed request to us by mail or fax (see below). Please include a copy of your state ID, driver’s license and/or passport, as we need to verify your identity prior to releasing a copy of your medical record.

The form must be completely filled out, signed and dated by the patient or legal representative. Please allow a minimum of 10 business days for processing the request after the form is received by UMC HIM. You may be charged 60 cents per page for copies of records for personal use or for insurance purposes in accordance with federal and state of Nevada regulations.

Requesting Amendment to Your Medical Records

If you would like to amend your medical records at UMC, please complete the Request for Amendment of Protected Health Information (click here for SPANISH version) and forward the completed request to us by mail or fax. Include a copy of your state ID, driver’s license and/or passport, as we need to verify your identity prior to releasing a copy of your medical record.
