Pre-registering Before Your Surgery

UMCSN Ambulatory Pre-Registration InformationThank you for choosing University Medical Center for your surgical procedure. Pre-registering for your procedure shortens the admission process and saves you time. Follow the guidelines below to expedite the pre-registration and appointment experience.

At least 72 hours before your surgery:

  • Call 702-383-2028 between the hours of 7:30am to 4:00pm to arrange a convenient time for you to come to the Ambulatory Surgery Center prior to your procedure and have the following information available:
    • Your insurance card(s)
    • Physician order, if available
  • Please allow approximately 2 hours to complete your pre-admission visit, which includes signing admission forms, visit with a pre-assessment nurse and complete pre-operative testing.
    • It is important to keep this appointment in order to facilitate your care on the day of surgery. This also gives us the opportunity to evaluate your lab and Xray results prior to your procedure, thereby ensuring your safety.
  • What to bring for Pre-Admission
    • Picture ID
    • Insurance card(s)
    • Physician orders
    • Physician instructions
    • Pre-Anesthesia Record (print and complete the form)
  • Insurance co-payment, deductible and/or deposits
    • Deductibles, co-payments and deposits are due at the time of service
  • For your convenience we accept cash, checks, Debit Cards, MasterCard, VISA, Discover and American Express
  • You may contact your insurance company using the phone number on the back of your card to determine what your financial responsibility will be
  • You may also contact us at 702.383.7396 to discuss your insurance benefits or make arrangements for a deposit if you have no insurance
    • Arrange for an adult to accompany you on the day of your surgery to drive you home

On the day of your surgery

  • Report to the Ambulatory Surgery Center 2 hours before your procedure, unless otherwise instructed by your physician
  • Follow all instructions provided by your physician and the pre-assessment nurse
  • Leave all jewelry and valuables at home except a means to pay your co-payment, deductible and/or deposit if you have not already done so
  • Dress in casual, loose-fitting clothing
  • Bring all forms given to you by your physician
  • Bring a copy of your Advanced Directive if you have one established
