Pre-registering Before Your Surgery

72 HOURS BEFORE YOUR PROCEDURE

At least 72 hours before your surgery:
• You have been instructed by your doctor to pre-register at UMC. Please plan on pre-registering at least 3 days prior to your procedure. Pre-registration can be done at Admitting just inside the UMC Main Entrance located on Shadow Lane and Charleston Blvd. which is open Mon.-Fri. 8am-4:30pm. The pre-admission process should take approximately 2 hours to complete depending on the complexity of your physician’s orders, which includes signing admission forms, discussing your health history with a pre-assessment nurse, and completing pre-operative testing. Please call 702-207-8295 to set up your pre-registration appointment.
It is important for you to pre-register in order to facilitate your care on the day of surgery. This also gives us the opportunity to evaluate your lab and x-ray results prior to your procedure which ensures your safety.
• You MUST bring the following for pre-registration:
Picture ID
Insurance card (s)
Physician orders (provided to you by your Doctor’s office)
Physician instructions (written by your Physician)
UMC Patient Packet with Pre-anesthesia Record completed including list of medications with dose and frequency taken.
Any legal documents such as Power of Attorney for Healthcare or Legal Guardianship papers that will be necessary to verify your ability to consent for another individual.
Pacemaker/other implanted device cards
• If your physician ordered pre-operative testing at another facility, please bring the results with you or have them faxed to (702) 383-3620 prior to the day of surgery.

• Insurance co-payment, deductible and/or deposits
You are required to pay your financial responsibility at the time of your pre-registration.
You should contact your insurance company using the phone number on the back of your card to determine what your financial responsibility will be.
• You MUST arrange for an adult to accompany you on the day of your surgery to drive you home. We do not allow post-operative patients to travel home unaccompanied by bus or cab.

24 HOURS BEFORE YOUR PROCEDURE

24 hours before your surgery:
• Do not eat or drink after midnight unless instructed differently.
This includes chewing gum, coffee, mints, ice chips, and water.
Undigested food/liquids in the stomach may cause complications and your surgery could be postponed or cancelled.
• Please bring a list of all medications you have been on in the last month including prescriptions and over the counter medications, vitamins, herbs, and diet pills. You should have discussed with your physician and surgeon which medications, if any, should be taken on the morning of surgery with a small sip of water.
• Refrain from smoking and use of tobacco products, illicit drugs or drinking alcohol.
• Pediatric Patients:
Please bring a diaper bag, bottle, Sippy Cup, or pacifier to comfort your child in recovery. Feel free to bring your child’s blanket or favorite toy for added reassurance.

DAY OF YOUR PROCEDURE

On the day of your surgery:
• Report to Ambulatory Surgery Center (1800 W. Charleston Blvd.) at the Charleston Blvd. entance 2 hours before your procedure, unless otherwise instructed by your physician. Refer to the Hospital Campus Map.
• Follow all instructions provided by your physician and the pre-assessment nurse.
• Bring the following: Glasses, Dentures, Hearing Aids with appropriate containers.
• If you are being admitted to the hospital please leave your overnight bag in the car.
• Shower or bathe using anitbaterial soap and brush your teeth (do not swallow any water) the morning of surgery to feel refreshed and to minimize the chance of infection.
• Leave all jewelry and valuables at home.
• Dress in casual, loose-fitting clothing.
• Bring your ID for identification purposes.
• Bring all forms given to you by your physician and your pre-op folder.
• Bring a copy of your Advanced Directive, if you have completed this document.
• Bring your method of payment, if you plan on paying the day of surgery.
