UMC now offers four types of appointments: First/Second Doses for Ages 12+, Pediatric Doses for Children Ages 5-11, Booster Doses, and Third Doses for Immunocompromised Patients. Please review the information below to ensure you select the correct appointment type.

Click Here to Schedule Your First or Second Dose (Ages 12+)
Patients ages 12 and older should select this option to schedule their first or second vaccine doses. Please note that UMC automatically schedules second-dose appointments for the Pfizer and Moderna vaccines.

Click Here to Schedule a Pediatric Pfizer Dose (Ages 5-11)
Children ages 5 to 11 are now eligible to receive first and second doses of the Pfizer COVID-19 vaccine. Please select this option to schedule an appointment for your child. The Pfizer vaccine is administered in two doses, scheduled 21 days apart. UMC automatically schedules second-dose Pfizer appointments for patients.

For those who previously received the Pfizer or Moderna vaccines, booster doses are available for everyone 18 and older, administered at least six months after the second dose. For those who previously received the Johnson & Johnson vaccine, booster doses are available two or more months after the initial dose.

Third doses of the Pfizer and Moderna COVID-19 vaccines are available only for certain immunocompromised patients (ages 12+ for Pfizer and 18+ for Moderna). Third doses must be administered at least 28 days after the second dose. Please click the link above for detailed eligibility criteria.
