Third Dose Registration

All COVID-19 vaccine appointments take place at the UMC Advanced Center for Health, located at 2231 W. Charleston Blvd.

Vaccination status may be disclosed as required by State or Federal law.

Please note that Third Doses are available only for certain immunocompromised patients. If you do not meet this criteria, please visit and select the appropriate appointment type.

Third Dose Eligibility Notice
Third doses of the Pfizer and Moderna COVID-19 vaccines have received Emergency Use Authorization for certain immunocompromised patients (ages 12+ for Pfizer and 18+ for Moderna). Third doses are currently available for eligible patients who previously received the Pfizer or Moderna vaccines and must be administered at least 28 days after the second dose. In order to receive a third vaccine dose, we ask you to attest that the following applies to you:

I have a moderately or severely compromised immune system. This includes people who have:

  • Been receiving active cancer treatment for tumors or cancers of the blood
  • Received an organ transplant and are taking medicine to suppress the immune system
  • Received a stem cell transplant within the last two years or are taking medicine to suppress the immune system
  • Moderate or severe primary immunodeficiency (such as DiGeorge syndrome, Wiskott-Aldrich syndrome)
  • Advanced or untreated HIV infection
  • Active treatment with high-dose corticosteroids or other drugs that may suppress your immune response

Please only proceed to scheduling if you attest that the above statement applies to you. When selecting an appointment, please choose an appointment offering the same vaccine you received for your first and second doses.

Please note that patients should talk to their health care provider about their medical condition and whether receiving a booster dose is appropriate for them.

Upon arrival, please be prepared to provide UMC’s registration staff with your vaccination card, government-issued ID, and insurance card.

Important Information for Patients Under 18
Patients younger than 18 years old are only eligible to receive the Pfizer vaccine. If you are younger than 18, please only select an appointment for the Pfizer vaccine and do not select a Moderna or Johnson & Johnson vaccine appointment.
